Athlete Information

Welcome to The Master's University. We are excited to have you compete on our teams this year. This page will inform you on what steps you need to complete within the Sports Medicine Department to be eligible to participate in activity.

SportsWare
SportsWare is the online injury tracking software used by the Sports Medicine staff. It also houses all our athletes' emergency contact insurance information and required documents.
  • If you are a new athlete, you will need to set up a SportsWare account following the information in the video below.
  • Every year, all athletes are required to update their information and resign documents.
  • When completing the insurance section, please make note of who is the correct policyholder.
  • Be sure to upload pictures of both the front and back of your insurance card.
  • All documents under the Attachments section are required, except the Health History Questionnaire, which is only required for returning athletes.

Wellfleet Insurance

Wellfleet insurance is the primary insurance policy that TMU offers to athletes without a primary insurance policy. When an injury occurs with this insurance, a specific form must be filled out called the Sports Injury Claim Form. Below are the instructions on how to find and fill out this form.

Physicals
If you are a first, third or fifth-year athlete at The Master’s University, you are required to receive a physical prior to arrival on campus. The two-page document is linked below. Both sections must be completed.
  • The physical paperwork must be completed by an MD, DO, PA, or NP. Physicals completed by a Chiropractor or Physical Therapist will not be accepted.
  • If you cannot get in with your regular doctor, most urgent care clinics will be able to perform a physical.
  • Any second or fourth-year athletes who are new to TMU will still be required to receive a physical.

Secondary Insurance
All student-athletes at The Master’s University are required to be covered by a primary insurance policy that provides coverage for emergent and non-emergent incidents in Southern California. If the primary insurance policy provided by a parent or guardian does not fulfill this requirement, it is highly recommended the student-athlete purchases an individual insurance policy through TMU. The secondary insurance of the athletics department will cover what the primary insurance does not, provided the guidelines are followed.
  • The secondary insurance only covers incidents that occur during an official practice, game, or direct travel to or from an official team activity.
  • Injuries that happen during a captains practice, intramural sports, or other non-official activities will not be covered by the secondary insurance.
  • Illness or general medical conditions are also not covered by the secondary insurance.
  • Self-referral to doctors not discussed with the sports medicine staff will not be covered by the secondary insurance.
Any questions regarding the secondary insurance policy should be directed to the sports medicine staff.
